As a Home Health Registered Nurse, you will provide hands-on care, management and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision.
Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives. Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist the physician in evaluating the patients level of function.

coordinate appropriate care, encompassing various healthcare personnel (such as Physical Therapists, Occupational Therapists, Home Health Aides and external providers).
The POC Registered Nurse in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations and agency policies.
In this role, you will be responsible to provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer on-going care and case management for each patient, provide necessary follow-up as directed by the clinical manager.
Performing everyday tasks in the home. Occupational Therapists/OTs use treatments to develop the daily living skills of their patients and the basic motor functions of patients, as well as to compensate for any loss of function that patient may be experiencing.
The Intake Assessment Coordinator in conjunction with the Program Director coordinates the pre-admission assessment process and provides clinical assessments for potential patients. Also, assists with community education with clinical issues and participates with community events such as health fairs.
