The Advanced Practice Provider, while providing quality care across the continuum in collaboration with physicians, nursing staff and other members of the care team, performs histories, physicals, and procedures as outlined in privileging agreement.

The Nurse Practitioner (NP) 2 works as part of the Clinical Operations Team, providing clinical assessment and care to assigned patients in their homes and in skilled nursing facilities; identifying quality improvement activities and planning individualized health care consistent.
