The patient care coordinator records multidisciplinary treatment plans, reviews treatment plans with patient, parent/guardians and acquires appropriate signatures, facilitates commitment hearings, and assists therapists in the completion of substance abuse assessments as needed.
In this position, you will collaborate with clinical and non-clinical staff and other members of the patients health care team to develop an individual plan of care to facilitate an immediate action plan to meet the patients current needs.
In this role, the selected candidate will serve as a consultant to other care providers for establishing appropriate care and services when care provided spans various sites/settings along the continuum.
In this role, you will provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy. Demonstrates problem solving and decision making abilities.
In this role, you will be responsible to actively participates in designing and implementing institutional program/initiatives to improve patient access to the medical center resources, quality of care, cost reduction efforts and patient satisfaction.
