In this role, the selected candidate will coordinate clinically complex patients care across a continuum; ensuring and facilitating the achievement of quality clinical and cost outcomes, negotiating, procuring and coordinating appropriate services and resources needed by the patients, and at key points, intervening to address and resolve issues/concerns.
In this position, you will be responsible for assessing and analyzing an injured employee to evaluate the medical and vocational needs required to facilitate the patients appropriate and timely return to work.
In this role, you will perform other duties as assigned In-person medical case management may be required to support Concierge locations, where a Nurse Case Manager meets with injured employees face to face following office visits at a provider's medical facility.
The Registered Nurse Case Manager serves as a member of the interdisciplinary care team. The RN Case Manager participates in the development and coordinates the implementation of the interdisciplinary plan of care.
In this role, the selected candidate will utilize regional population stratification information and processes to identify appropriate members for enrollment into case management in collaboration with clinicians and health care team members of the medical home.
As an RN Case Manager, you will have the ability to work at the top of your licensure while working one-on-one with your clients to provide them customized care. Under the guidance of your supervising physician, you will develop plans of care and utilize nursing theories, skills and techniques to provide quality care to your clients on a daily basis.
