In this role, you will utilize regional population stratification information and processes to identify appropriate members for enrollment into case management in collaboration with clinicians and health care team.
The nurse case manager is responsible for performing case management within the scope of licensure for patients with complex and chronic care needs by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize health care across the care continuum and ensuring patient access to services appropriate to their health needs.
As an RN Case Manager, you will have the ability to work at the top of your licensure while working one-on-one with your clients to provide them customized care. Under the guidance of your supervising physician, you will develop plans of care and utilize nursing theories, skills and techniques to provide quality care to your clients on a daily basis.
The RN Case Manager participates in the development and coordinates the implementation of the interdisciplinary plan of care; plans, organizes, and provides nursing care to the terminally ill and their families.
