In this role, the selected candidate will collaborate with the patient and support system(s) to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the inter/multi-disciplinary team (IDT).
In this role, you will provide patient-centered social work services to the most medically fragile Providence Health Plan (PHP), at-risk patients/clients in a variety of residential settings to enable patients.
In this role, the selected candidate will be responsible for the provision of direct social work service in Home Health and/or Hospice utilizing the professional healthcare process of assessment, problem identification/planning, action/intervention and clinical goal/outcome evaluation.
In this role, the selected candidate will independently provide comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems.
The Clinical Social Worker coordinates the development and implementation of a patient care/treatment plan through use of diagnosis and clinical assessment of psychiatric, chemical dependency and psychosocial needs as they relate to medical and behavioral care needs. Provides disposition and discharge planning, risk assessment, case management, and coordination of services to provide continuity of care.
