Responsible for coordination and continuity of care working with the PCP's, referring physicians, patients, CCF cardiology staff, staff surgeons and their office staff, other nurse practice managers (NPM), as well as the scheduling officers, preop/TCI/postop nurse clinicians, to assure continuity of care and appropriate concern for individualized presurgical and/or postsurgical needs.
