In this role, the selected candidate will provide safe and effective delivery of home dialysis training and education to patients with End Stage Renal Disease (ESRD) who have selected a home modality as a dialysis therapy.
In this role, you will document all relevant data including physician orders, lab results, vital signs, and treatment parameters, and patient status and any treatment related data appropriately, and enters into the appropriate medical information system.
The selected candidate maintains appropriate skill level in the operation of all home dialysis therapy-related equipment, including the ability to troubleshoot equipment problems with the patient via telephone.
The selected candidate will provide a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.
The selected candidate will be responsible for both individual and group education to promote core health improvement programs including smoking cessation, adult immunizations, heart health, cancer prevention and detection, and wise self-care.
In this role, you will assumes operational responsibility for the development of patient and staff education within designated center(s) population(s). Promotes the application of nursing clinical expertise in a specified patient population.
