In this role, you will manage the human resources functions for the Social Services unit to develop a team of skilled, informed, professional staff, including recruitment, training, supervision, evaluation and scheduling in order to achieve objectives and assure the provision of services.
In this role, you will ensure adequate mental health, medical, social services, housing placement and substance abuse services for all clients; communicate with referral agencies on admission criteria and bed vacancies.
In this role, the selected candidate will work with patient/resident, family members/significant others, and interdisciplinary care team through care planning and utilization management throughout the course of the stay to identify strengths and needs to ensure an appropriate discharge plan is formulated.
