As Case Manager you will facilitate the achievement of patient wellness and autonomy through advocacy, assessment, planning, communication, education and resource management. ensure safe, effective, patient centered, timely and efficient patient care.
The Case Manager works with insurance carriers, medical care providers, attorneys, employers and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carrier.
In this role, you will provide case management to members who are identified (specifically members being discharged from hospital admissions) in accordance with companies Medical Management policies and procedures.
Our RN Case Manager, plays an essential role in the patient experience by developing and performing an age-specific plan of care for patients from their admission to the hospital, and throughout their hospital stay.
In this role, you will evaluate and perform ongoing assessment and revise initial written plan of care with Interdisciplinary collaboration weekly or as the needs and conditions of the patient/family change.
