The Speech Language Pathologist is responsible for the planning and provision of speech language rehabilitation services to patients in their home and to work with the patient, family and/or others to provide training and/or assistive devices for speech and language rehabilitation.
In this role, the selected candidate will provide screening, identification, assessment, diagnosis, treatment, and intervention for children with communication disorders; maintain an accurate record of services provided, submit evaluation reports and monthly progress reports in a timely manner.
In this role, the selected candidate will be responsible for the evaluation, diagnosis, and implementation of appropriate treatment program designed to habilitate or rehabilitate a variety of communication disorders.
In this role, the selected candidate will provide direct resident treatment in accordance with an established plan of care under a physicians order and issue specialized adaptive equipment to improve residents ability to communicate and swallow.
In this role, the selected candidate will adhere to all policies of company, including job descriptions, certifications requirements, mission statement, HIPAA, confidentiality standards and patient rights.
In this role, the selected candidate will record information on the initial evaluation, treatment, progress, and discharge of clients;provide communication instruction to Residents with limited English proficiency.
In this role, you will plan, establishes, and exercises proper treatment program in accordance with the physician's referral, periodically, reviewing the resident's / patient's program and response and making recommendations to the physician relative to the need of continuing or discontinuing the Speech therapy treatment.
