The Community Outreach Coordinator is: responsible for building and maintaining relationships with a wide range of referral sources to ensure that SCO's Nurse-Family Partnership program, a nurse home visitation program for low-income first time mothers beginning in pregnancy and continuing until the child's 2nd birthday, is effectively utilized within targeted Brooklyn communities.
In this role, you will assist in the successful implementation of truefacets communications function, with a strong emphasis on the execution of integrated communications plans where media outreach is a major element, as well as assist in various projects as assigned by the head of brand & communications.
This position is project-based and will terminate three months after the start date. Compile and distribute daily clips for the Foundation, including news stories about the Foundation, President Clinton, Chelsea Clinton and issues relevant to the Foundations work.
This position requires an excellent understanding of effective communication strategies, the ability to measure campaign success, an excitement for beating the control to increase engagement, creative graphic design/layout skills, and a strong commitment to articulating the company culture through communications.
In this role, you will support for research grant submissions, progress reports, and renewals, including significant compilation of materials, proofing and editing; requires gathering and applying knowledge of specific application and submission guidelines used by state, federal, private and other sponsors. With respect to these activities, works in coordination with departmental grant administrators. Participates in training opportunities relevant to the position.
The Community Care Coordinator-RN (CCC) is accountable for the management and coordination of the patient's plan of care in the primary care office, extending into the community, and transitioning the patient through acute care and post-acute care services as needed.
