In this role, the Social Worker will be responsible for assessing, counseling, intervening, implementing and educating the patient, family/significant others to facilitate movement throughout the continuum of care.
As a member of the interdisciplinary team, assesses patients' psychosocial status, strengths and areas of need that may affect rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment.
In this role, the selected candidate will act as an advocate for both the client and the family to navigate the patient through the community system; develop and maintain contracts with public and private agencies as resources for the patient.
The Social Worker 2 is responsible for planning, organizing and implementing social work services to patients and families, as well as coordinating the discharge plan with the nurse case manager, multidisciplinary team and community agencies as necessary in a very busy ER.
In this role, the successful candidate will be responsible for cordinating with Child Psychiatrist and Program Manager in assessing data to refer for consultation and/or to provide care management services to linkages to care.
