In this role, you will provides support, interacts, and serves as a clinical resource to patient care staff. Functions as a resource for policy and procedure interpretation. Utilizes age appropriate patient care interventions.
In this role, the selected candidate will manage performance including: setting goals, clarifying job expectations, monitoring performance progress, providing feedback, recognizing performance, developing skills and addressing performance issues related to work and our Core Values using corrective action.
In this role, you will assure the attending physician is updated on the condition and needs of patients, assure that patient care plans reflect current goals of care and interventions. In this role you are empowered to be a leader because you are the liaison between residents, families, doctors and other members of the healthcare team.
In this position, you will coordinate timely and accurate patient assessments, interventions and evaluations with supporting documentation in the patients record to assure that desired resident outcomes and standards of patient care are met.
