Responds appropriately to emergency situations (Cone Health Codes) and demonstrates knowledge of how to respond. Always uses appropriate safety devices and techniques (examples: using safe lift equipment and needle safe devices; pushing carts; correctly lifting copy paper).
Supervise, assess and monitor patients during cardiac and pulmonary rehab including exercise therapy and education sessions. Coordinate patient care and respond to emergency situations. Evaluate and recruit potential candidates for cardiac rehabilitation.
The nurse uses clinical skills to coordinate, document and communicate all aspects of the utilization management program. Utilizes clinical experience and skills in a collaborative process to assess, plan, implement, coordinate, monitor and evaluate options to facilitate appropriate healthcare services/benefits for members.
Provides safe and effective training and oversight of the delivery of home dialysis therapy to the patient in compliance with standards outlined in the Home Peritoneal Dialysis Policy and Procedure Manual, or/Home Hemodialysis Policy and Procedure Manual , as well as regulations set forth by the Corporation, state and federal agencies.
In this role, you will provide and coordinate patient care, educate patients and the public about various health conditions, and provide advice and emotional support to patients and their family members.
Responsible for the daily management of an assigned patient caseload, ensuring timely care and documentation to meet the patent needs/services as prescribed in the established plan of care and per MD orders.
