Under a physicians supervision, be responsible for the delivery of a broad range of primary care medical services, including taking medical histories, conducting physical examinations, ordering diagnostic tests, administering treatments (e.g., suturing and minor surgery), monitoring patients, prescribing medications, assisting physicians, and counseling and educating patients within our PACE facility.

The Nurse Practitioner will deliver acute and/or routine problem-focused care to patients under the general supervision of the Primary Care Physician Director. In collaboration with the primary care Physician Director, the Nurse Practitioner will provide medical management, patient education, and care coordination to patients desiring care for non-life threatening, acute care needs.
The Nurse Practitioner provides direct patient care for patients under medical direction of the primary supervising physicIan and/or Medical Director. Arranges follow-up care and/or referral for patients, as necessary.
