Maintain accurate and current notes in the paper medical records, if created, and electronic medical records of all patients seen, as appropriate and produce reports of evaluation or treatment, as required.
The Licensed Social Worker will assist in the planning, developing, organizing, implementing, evaluating, and directing of the Social Services Department within the facility in accordance with current existing Federal, State and local standards as well as our established policies and procedures to ensure that the medically related emotional and social needs of patients and/or residents are met and maintained on an individual basis.
The Social Worker is responsible to provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental, and psychosocial well-being.
In this role, you will be responsible for the application of social work procedures and techniques, including interviewing, behavioral assessment, and evidenced based therapies in the evaluation, diagnosis, and treatment of psychological and neuropsychological disorders using individual, family, and group psychotherapy, couples therapy, and alcohol and drug treatment evaluations.
In this role, you will Assess patient knowledge of kidney disease for barriers that may affect adherence to treatment. Works with patient, family and health care team to provide education tailored to the patients learning style and needs.
Provides counseling and crisis intervention as an immediate support to families, patients and significant others, coping with difficult situations within the acute care setting, as well as a preventative intervention to assist patients and families in improving functional problem solving.
