This position is responsible for evaluation, referrals and coordinating patient/family needs. Under physician orders, the Registered Nurse is responsible for identifying and coordinating patient/family needs and for providing preventative and supportive care to the terminally ill patient/family unit.
In this role, the selected candidate will relate effectively with other shifts for continuity of care; maintains satisfactory relations with other departments and nursing units; participates in all phases of education, maintenance of records and updating of policies, procedures and skills of personnel.
The RN performs functions of assessing; planning implementing and evaluating care for assigned patients on unit during shift. The RN is responsible for managing all assigned personnel, supplies and equipment.
The Primary functions are to plan, develop, implement, evaluate, organize and direct home care services with the interdisciplinary team including physician, patient/family, and referring agency; and administer skilled nursing care for clients of all ages in their place of residence and assume the responsibility for the total provision of care.
In this role, you will oversee and provide primary care, including all clinical care, palliative care, and symptom control and identify other physical, psychological, social and spiritual needs for assigned patients.

initiating the appropriate intervention and support for the patient and family upon admission to VITAS and provides a continuously appropriate, comprehensive and responsive plan of care.
