As a Home Health Occupational Therapist Assistant, you will provide therapy services planned, delegated and supervised by the qualified Occupational Therapist in accordance with the patients Plan of Treatment.
In this role, you will act as a liaison and maintains contact with patients, Physical Therapist, Clinical Manager, other members of the healthcare team and the community in assuring effective care coordination and appropriate resource referrals.
In this role, you will be responsible for meeting the Standards of Practice, which includes managing all assigned personnel, supplies, and equipment on the unit, and for the promoting collaboration with all members of the health care team.
