Provides ongoing education to Medical Services Department and Physicians on locally available community resources and develops and maintains effective relationships with staff in key community resource agencies.
Conducts initial assessment of all new patients to evaluate social, emotional and physical needs by applying social work theories and techniques; interviews and evaluates patients and family members. Identifies factors which may interfere with obtaining maximum benefits from treatment.
Assesses, develops and coordinates a clinical treatment program for Health Plan members with acute or chronic psychiatric disorders. Consults with staff regarding diagnosis, strengths and deficits of member, as needed or appropriate.
Responsibilities are psychosocial assessment, crisis intervention, consultation, education, community linkages and supportive services for identified patients, families and staff for the Kidney Transplant Service.
The Medical Social Worker II serves as an integral member of the healthcare team providing assessments, coordination, treatment planning, information and referral to community resources and other social work services to meet the complex needs of patients and families in the hospital and clinic settings.
