In this role, you will be responsible for conducting an initial nursing assessment of a potential hospice patient and reporting the patients condition to the Hospice Medical Director using the applicable Local Coverage Determinations.
The RN functions within the full scope of licensure to implement the nursing process. The RN will assess patient and family needs, develop a plan of care based on assessment, direct implementation of the plans of care for a group of patients, collaborate with the charge nurse and care team, and evaluate effectiveness of the care plan.
In this role, you will provide a wide range of nursing services, including conducting the initial evaluation visit, regular re-evaluations, initiating the plan of care with necessary revisions, and specialized care to adult and geriatric, patients in their home.
Provide direct and indirect patient care to clinic patients and their families utilizing the nursing processes of comprehensive patient assessment, development and implementation of nursing care plans and patient education, and on-going evaluation.
Provide immediate direction to other members of the nursing care team to assure the appropriate provision of nursing services.
