In this role, the selected candidate will utilize professional medical social work skills to assess patient and family needs and provide proactive individualized planning that optimizes quality of care across the continuum.
The LGSW coordinates patient discharge planning, complies with required standards for medical/legal responsibilities, and serves as a resource to other allied and administrative staff within the Section of Social Work and the Department of Nursing.

Assists the health team in understanding the significance of social, economic, emotional and family factors in relation to the customer's illness, treatment, recovery or dying processes.
In this role, the selected candidate makes frequent walk through visits with residents, interacting with residents, and making observations and conservation on a casual basis to establish rapport with residents and their families.
