The Social Worker provides, coordinates and administers social services for our health care customers, patients and residents in accordance with applicable standards, guidelines and regulations. Working within a multi-disciplinary team environment, the Social Worker completes bio-psychosocial assessments, develops individual plans of care, coordinates care and services, discharge planning activities and other services that are complex in nature.
In this role, the successful candidate will provide support and counseling to patients/families experiencing and/or anticipating issues to adjusting to illness, catastrophic diagnoses, changes in living situations and bereavement.
Responsible parties and the interdisciplinary team. The social worker iI will be engaged in program assessment, development and implementation, policy formation, committee work, performance improvement projects and education facilitation.
