Provide education to senior leadership, regional leadership, consultants and community personnel to ensure all are informed of changes in expectations, trends, policies and practices related to MDS, Point Click Care, medical record documentation and delivery of routine and specialized nursing cares .
In this role, the selected candidate will conduct total office call and/or in-service sessions to educate physicians, nurses, infusion centers, and office staff on patient services and patient identification.

Triage urgent/emergent same day patients for the team. Identifying acute patient needs and high acuity patients, identifying veterans at risk for uncoordinated care and mobilizing a care team for optimal patient outcomes.
