In this role, you will be responsible for providing IV nursing care in the home or ambulatory setting, performing in - hospital discharge assessments and occasional IV hook - ups, teaching patients about their home infusion therapy, and providing ongoing monitoring of their treatment.
The RN provides leadership through activities such as preceptor role, informal leadership roles, and quality improvement efforts. The RN delegates patient care according to skill level, experience, patient acuity, fiscal accountability, and adequacy of resources.
Triage urgent/emergent same day patients for the team. Identifying acute patient needs and high acuity patients, identifying veterans at risk for uncoordinated care and mobilizing a care team for optimal patient outcomes.
Identifies opportunities for connecting members to group related benefits; e.g. Nurse Line, Employee Assistance Program, Dedicated Nurses, Dedicated Behavioral Health Clinical Specialists or other specialists within BCBSM Health Management Division.
