This individual is responsible for the medical case management of work related injuries which includes assessment, planning, coordination, implementation and evaluation of. injured/disabled individuals.
In this role, the selected candidate will establish and maintain good working relationships within the Case Management department, with other HealthPartners departments, and with other health team participants.
engaging the member, the members provider care team and Medica care management resources with the objective of executing a care plan that leads to the members self-management capabilities and defines accountability for the members supporting network of resources.
