Plans, organizes and imlements social work services for the patients and their families/significant others in their homes. Acts as a counselor, advisor and resource person to patients in accordance with the medical plan of treatment, standards, and the National Association of Social Workers standards for social work in the health care setting.
Primary function is to provide coordinated care in the home to hospice clients of all age groups and perform psychosocial assessments, analysis, counseling, and referrals to meet the needs of the patient/family.
Conduct psychosocial assessments with patients and their families. Other duties include documenting in the medical record psychosocial histories, discharge planning, patient contacts, treatment updates.
