The relief LMSW Social Worker is a professional who collaborates with physicians, health care team members, and patient/families to coordinate care for specific patient populations across the continuum and between care settings during an episode of illness.
The Social Worker is the member of the interdisciplinary team whose primary function is to provide diagnostic evaluations of the psychosocial problems of the patient/family unit, defining social service goals for alleviating these problems, and helping to provide both the counseling and casework needed to meet these goals.
As a member of the interdisciplinary team, assesses patients' psychosocial status, strengths and areas of need that may affect rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment.
In this role, you will be responsible to provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental, and psychosocial well-being.
In this role, you will provide a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.
In this role, you will communicate with patient, family, relevant professionals, programs, agencies and others to help client secure service needs, make preferences known, and determine appropriate placement within the facility and in the community.
The selected candidate will be responsible for supportive counseling of patients and their families suffering from acute emotional distress (i.e. adoptions, physical disabilities, terminal illness, abuse and trauma).
In this role, you will be responsible to
provides psychosocial services to patients treated by the facility including in-center and home dialysis patients (if applicable) utilizing social work theory of human behavior and accepted methods of social work practice. Works with the health care team to promote positive adjustment, rehabilitation and improved quality of life for our patients.
In this role, the selected candidate plans, organizes and implements social work services for the patients and their families/significant others in their homes. Acts as a counselor, advisor and resource person to patients in accordance with the medical plan of treatment, Tenet standards, and the National Association of Social Workers standards for social work in the health care setting.
