In this role, you will perform and document H&P's on new patients placed in Observation status, completing of medication reconciliations, and collaborates with the primary care physician, consultants and counselors to clarify patient and family goals, identify achievable therapeutic goals, and facilitate care in coordination with the interdisciplinary team.
In this role, the selected candidate will provide basic nursing duties such as the recording of vital signs, audiograms, vision testing, urine collection, blood collection, spirometry, respirator fit testing, injections and basic first aid is required.
Performing invasive diagnostic and therapuetic procedures based on the needs of the specific patient population as determined collaboratively with the physician and as approved by the Credentialing Committee.
The Nurse Practitioner collaborates with the interdisciplinary hospice team in the development of the comprehensive plan of care, assessment of patients, and participation in the identification of eligibility for Hospice Medicare Benefit. The Nurse Practitioner collaborates with the Hospice Medical Director.
The Nurse Practitioner or Physician Assistant will perform health assessments, medical histories and physical examinations; prepare physician orders, see follow-up patients independently; request/evaluate diagnostic tests; prescribe medications; provide patient education and instruction per supervising physician.
