In this role, the selected candidate will be responsible for service delivery and outreach to migrant and/or homeless populations in Palm Beach County at the existing health centers, along with other community sites, as directed by the Clinic Administration.
The Social Worker position provides group, individual and family therapy as well as organizing and directing treatment resources, discharge planning, patient progress, compliance with regulatory and accreditation standards and assuring that related issues are effectively addressed and communicated to the patient, family and others involved in the treatment process.

In this role, you will serves as the primary child development resource for families in the pediatric practice, delivers direct services to families with infants and/or young children, and coordinates the efforts of all members of the team at each site.
The primary responsibilities will include crisis intervention, forensic and specialized interviewing, report writing, case management, coordination of medical services, public speaking, training, and utilization and coordination of community resources.
