The Registered Nurse (RN) Care Coordinator works within an interdisciplinary team to coordinate care for a defined population of patients. The goals of care include supporting improvement in health outcomes.
In this position, you will be responsible to ensure the delivery of high quality care to our residents through the supervision of the direct care staff. Responsibilities will also include performing treatments, coordinating physician's visits, processing orders and completing necessary documentation.
This position also works as a flexible RN resource within the Primary Care teams. In addition, they may participate in planning, implementing, coordinating and evaluating patient care activities, staff education, and management activities as delegated by the Clinic Manager or Clinical Services Leader.
