The Ambulatory Medical Home Pharmacist is responsible for providing comprehensive pharmaceutical care to patients in the primary care clinics. The Pharmacist evaluates, identifies and solves medication related problems and acts as a liaison between patients and patients families and other health care team members regarding medication issues.
The Clinical Formulary Pharmacist must be proficient in online literature retrieval and evidence-based medicine. As a Clinical Formulary Pharmacist, you will be the clinical research and analysis for the drug classes/drugs related to the formulary and drug lists.
The Formulary Management Pharmacist will provide clinical subject matter expertise and support for internal and external clients as well as additional formulary related clinical activities to meet organizational and marketplace needs.
