The Case Management Department provides our patients and clinical staff with support to determine the appropriate setting of care and appropriateness of hospitalization, as well as to assess post-hospital needs.
In this role, you will be responsible for working with adult and geriatric patients, families, medical staff and community agencies to determine the appropriate setting of care and appropriateness of hospitalization, as well as to assess post hospital care needs.
In this role, you will be responsible for supervising, implementing, coordinating, and managing patient care through interpersonal contact with patients, families, nursing staff, and others on his / her respective unit in accordance with all applicable laws, regulations, and Life Care standards.
In this role, the selected candidate will be responsible for coordination of care across the continuum during the patients acute, chronic and long-term stages of illness for a defined patient population.
In this role, the selected candidate will educate providers, members and their families on behavioral health issues including symptoms, relapse prevention, stress reduction, lab work, and healthy lifestyle choices.
