The successful candidate will be responsible for assessing the psychosocial needs of patient/family, crisis intervention, short-term counseling, referral to appropriate rehab and community programs and agencies. Both groups of patients are high utilizers.

In this role, you will be responsible for
supervising up to 15 employees both exempt and non-exempt - working on a single project or task. Technical difficulty/complexity of assigned task/project may also affect level selection.
In this role, you will coordinate service management activities with the primary care provider, UM/CM staff, medical case managers, mental health professionals, chemical dependency treatment providers, public agencies, and other providers as required.
The Inpatient Social Worker, in collaboration with the clinical team and medical provider, provides patient and family advocacy, discharge planning coordination, and psychosocial intervention for the high risk inpatient.
