The clinical manager provides supervision, instruction, guidance, and evaluation for a group of clinical nurses, social workers, physical therapists, occupational therapists, speech/language pathologists, therapy assistants, case managers, home health aides, and other clinical support staff in accordance with agency, state , and federal standards.

The CCM understands and applies principles of population health management to identify patients with uncontrolled chronic conditions and/or rising risk indicators and adjusts patient assignments accordingly.
In this role, you will be responsible for the management, direct supervision and coordination of clinical and/or nonclinical management staff, including utilization management and intensive care management staff, providing expertise in designated programs.
