In this role, you will be responsible to provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer on-going care and case management for each patient, provide necessary follow-up as directed by the clinical manager.
Coordinate total nursing care for patients, participates in patient and family teaching and provides leadership by working cooperatively with other nursing staff and multidisciplinary team members in maintaining standards of professional nursing practice.
Provides patient care to patients undergoing Radiology Interventional procedures, including pre-procedure work up, patient monitoring, conscious sedation as required, and assists with post procedure recovery.
As a Home Health Registered Nurse, you will coordinate appropriate care, encompassing various healthcare personnel (such as Physical Therapists, Occupational Therapists, Home Health Aides and external providers).
Services are highly individualized and are delivered by a mobile, community based, multi-disciplinary team of highly dedicated staff who work with people to meet their unique rehabilitation needs, improve functioning and enhance their roles within the community.
In this role, the selected candidate will provide day to day guidance, support and direction to direct patient care staff, providing informal feedback on an ongoing basis and formal feedback input for the annual performance evaluation.
