The Staff Nurse helps coordinate and administer hospice care for patients, families and caregivers through the utilization of hospice resources. The primary staff is experienced in nursing with an emphasis on community health education and experience.
As a Home Health Registered Nurse, you will coordinate appropriate care, encompassing various healthcare personnel (such as Physical Therapists, Occupational Therapists, Home Health Aides and external providers).
Coordinate total nursing care for patients, participates in patient and family teaching and provides leadership by working cooperatively with other nursing staff and multidisciplinary team members in maintaining standards of professional nursing practice.
Provides patient care to patients undergoing Radiology Interventional procedures, including pre-procedure work up, patient monitoring, conscious sedation as required, and assists with post procedure recovery.
In this role, you will be responsible to provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer on-going care and case management for each patient, provide necessary follow-up as directed by the clinical manager.
In this role, you will communicate and documents interventions and patient response; contribute to an environment that is respectful, team-oriented, and responsive to the concerns of the staff, residents and families.
