In this role, you will oversee the frequency of visits for the episode, in addition to performing visits and completing coordination of client care. He / She will also attend weekly case conference and monthly case manager meetings.
This position is accountable for identifying and developing innovative actions to meet the needs of the member from both the health care and psychosocial / socioeconomic dimensions of care as well as taking action for provision of services to meet those needs.
In this role, the selected candidate will be overseeing population management activities with the Care Coordinators which includes addressing quality indicators that are out of range and assisting patients to reach targets.
