This position provides direct patient care relative to physical diagnosis and therapeutic management for high acuity patients in the inpatient and/or ICU setting. Works closely with faculty, fellows, residents and nursing staff to develop and coordinate a plan of care to meet the needs of the patient and their families. Off-hour, weekends and holiday coverage are often required.
The Nurse Practitioner (NP) works as part of the Clinical Operations Team, providing clinical assessment and care to assigned patients in their homes and in skilled nursing facilities; identifying quality improvement activities and planning individualized health care consistent with Matrix Medical Network policy protocols.
Performs enrollee clinical assessments according to established protocols visiting long term care facilities at least on a monthly basis to perform ongoing assessments, and more frequently based upon enrollee needs.

The Nurse Practitioner (Adult Diabetes) will serve as an integral member of the multidisciplinary healthcare team to provide quality, evidence based diabetes care, to an inpatient and outpatient adult population.
Establish communication channels with staff, physicians, contractors and consultants to ensure that they receive information on internal and external issues which effect the operations of the assigned areas as well as provide advice, assistance and service that aids them in attaining their related objectives.
