The Hospice RN Case Manager is a skilled professional member of the Interdisciplinary Team, responsible for implementing the nursing component of the plan of care and coordination of other clinical and support services. The position also provides direction and supervision to hospice aides and serves as the primary liaison with patients physicians and ancillary health providers.
