In this role, the selected candidate will perform a comprehensive nursing assessment upon admission of the patient including physiological, psychological and environmental factors as well as health history.
The case manager is accountable for coordinating and facilitating the management of care for an assigned group of patients to ensure provision of a well coordinated individualized care experience through the delivery of outcome oriented patient care within a fiscally responsible framework.

Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.
Responsible for working with Emergency Department staff to create, educate and implement substance abuse processes, treatment modules and evaluations for ED patients presenting with substance abuse concerns.
