Performs the primary functions of assessment, planning, facilitation and advocacy, which are achieved through collaboration with the client and other health care professionals involved in the clients care.
This individual is responsible for the medical case management of work related injuries which includes assessment, planning, coordination, implementation and evaluation of. injured/disabled individuals.
Conductinga comprehensive assessment of the clients health and psychosocial needs,including health literacy status and deficits, and develops a case management plancollaboratively with the client and family or caregiver.
In this role, you will develop an appropriate patient-specific plan of care to include short and long term goals, objectives and actions. Coordinates, collaborates, and obtains approval of the plan among the patient, family/caregiver, primary provider and other members of the healthcare team.
The Registered Nurse, Hospice Case Manager plans, organizes and directs hospice services for our hospice patients; the CM provides hospice care and is skilled at assessing pain and other complex symptoms at the end of life.
In this role, the selected candidate will be responsible for continuous review of all aspects of every patient on his/her caseload to include: appropriate utilization of services; ensuring continued skilled need; monitoring of homebound status; review of documentation in the medical record; maintenance of interdisciplinary communication and discharge planning.
