In this role, you will be responsible for the developement and maintenance of strong relationships with physicians, and staff in their efforts to deliver palliative/comfort care to the seriously or terminally ill patient.
Determine QA/QC protocols for each test that will effectively monitor all aspects of the testing based on accrediting agency requirements. Ensure that all personnel who are performing any laboratory testing have received adequate training and competency is assessed periodically.
The RN CASE MANAGER is responsible for Utilization Management, Quality Referrals, and Discharge planning activities on designated units within the hospital. Must be able to assess discharge-planning needs of a diverse cultural and ethnic population.
The point of care coordinator plans, coordinates and directs activities for point of care testing and directs the implementation of new procedures. Also provides technical assistance and training of personnel performing point of care testing in all areas at both hospitals.
In this role, you will build meaningful relationships with a specified number of seniors and their families as you provide assistance with activities of daily living, attend to individual care needs and get to know their unique preferences and personalities.
In this role, you will be responsible for patients records to collect data to carefully understand the needs of the patient by scrutinizing their background history, understanding their current needs, and arranging for their wellbeing.
