In this role, you will be responsible for patient assessment, patient care planning, and the provision of quality nursing care to an assigned group of patients for a defined work period. Provides direction and supervision to LVN/LPNs, nursing assistants, unit secretaries and other clinical/clerical associates.
Develop and maintain the audit process and tools related to: authorizations, appeals, quality events, and case management in CCMS; interrater reliability related to InterQual; and data entry into the credentialing subsystem.
Ability to identify scope of practice issues, prioritize duties and make effective decisions concerning patient care. Demonstrate communication and interpersonal skills, both orally and in writing. Ability to perform multiple tasks, often under stress.
Provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer on-going care and case management for each patient, provide necessary follow-up as directed by the clinical manager.
Must possess effective decision making, assessment and critical thinking skills. The sensory and communicative activities required for this position include feeling, hearing, seeing, smelling, and speaking.
In this role, you will provide and coordinate patient care, educate patients and the public about various health conditions, and provide advice and emotional support to patients and their family members.
