The Clinical Manager, under the direction of the Nurse Manager or Department Director, plans, organizes, staffs and directs patient care on a daily operational basis in accordance with the Ohio Nurse Practice Act.
Collaborates with the Care Team and follow up appropriately to ensure smooth transition of care for patients treated in a facility (inpatient or emergency department), by a specialty physician, or by another health care provider.
Performs clinical review of the appropriateness and quality of behavioral health services, applying established national decision support criteria, medical mutual policies and guidelines, individual patient considerations, and clinical judgment to determine appropriateness of services and procedures. Ensures compliance with policy and procedure identifies and refers potential quality of care and utilization issues
Responsible for providing strategic and operational support for care transitions strategic initiatives across kettering health network & kettering care coordination network (khn/kccn), to enhance the patient experience with coordination of care in the hospital setting and across the care continuum.
