Responsible For Providing Iv Nursing Care In The Home Or Ambulatory Setting, Performing In-Hospital Discharge Assessments And Occasional Iv Hook-Ups, Teaching Patients About Their Home Infusion Therapy, And Providing Ongoing Monitoring Of Their Treatment.
In this role, you will provide and coordinate patient care, educate patients and the public about various health conditions, and provide advice and emotional support to patients and their family members.
In this role, you will be responsible for providing IV nursing care in the home or ambulatory setting, performing in - hospital discharge assessments and occasional IV hook - ups, teaching patients about their home infusion therapy, and providing ongoing monitoring of their treatment.
In this role, you will assess and coordinate patient's discharge planning needs with members of the healthcare team. The selected candidate will provide education and resources to patient and / or patient's family based on clinical implications.
