In this role, you will be responsible to organize and conduct medically prescribed physical therapy programs to restore function, prevent disability following disease, injury or loss of a body part, and helps patients reach maximum performance levels.
In this role, the selected candidate will provide Physical Therapy Services to patients by assisting in the assessment, treatment, program planning and implementation, related documentation and communication.
The Home Health Physical Therapist is a licensed therapist possessing special knowledge of care practices and regulations required for provision of comprehensive care to patients in their home. Physical Therapists in a full-time status are responsible for managing the care plan for a group of assigned patients; coordinates with other disciplines and departments while maintaining accountability for care provided.
Plan and administer prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses and functional disabilities to attain highest level of physical function.
The Physical Therapist assesses the need for, develops, and delivers physical therapy programs to facilitate rehabilitation. The Physical Therapist identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of patients.
He/she has clinical and/or functional knowledge of the position for which they are accountable either directly or indirectly through delegation of tasks. Incumbent is accountable to the Administrative Team.
