Primary functions are to plan, develop, implement, evaluate, organize and direct home care services with the interdisciplinary team including physician, patient/family, and referring agency; and administer skilled nursing care for clients of all ages in their place of residence and assume the responsibility for the total provision of care.
The Registered Nurse (RN) utilizes the nursing process to coordinate the interdisciplinary approach to patient care. He/she provides safe, therapeutic care in a holistic and systematic way. He/she integrates knowledge, skills and experiences to meet the needs of patients and families through the continuum of care
Identifies and appropriately uses available resources in meeting the health care and health education needs of patients and families, including but not limited to consultation with the pediatrician, other physicians, and allied health professionals.
In this role, you will be responsible for promoting and restoring patient's health by completing the nursing process; collaborating with physicians and team members; and providing physical and psychological support to patients and their families.
Cares for medical and surgical cardiac patients undergoing antiarrhythmic therapy, invasive treatments or procedures, cardiovascular surgery and patients with pulmonary disease, who require an intermediate level of care.
In this role, you will provide and coordinate patient care, educate patients and the public about various health conditions, and provide advice and emotional support to patients and their family members.
In this role, you willensure nursing practice provided is within boundaries of State guidelines and all nursing recordings are in legible SOAP format to include the date, time, signature and discipline.
