In this role, you will collaborate with physicians, patients and their families, nurses, therapists, social workers and other members of the patient care team to develop a patient specific care and treatment plan. The selected candidate will assess, monitor, document and report progress of patient's health and condition using required documents, reports and established timelines.
In this role, the selected candidate will be responsible for providing comprehensive work place medical and/or wellness services, which may include patient triage/evaluation/referral into the health care system, phlebotomy, immunization administration, BLS instruction, hearing testing and other testing/duties as company-specific/contracted needs arise per guidelines.
In this role, you will act as a case manager in coordinating care for the behavioral health / psychiatric patient. Regularly reviews treatment planning under the direction of a physician or psychiatrist which includes nursing actions and interventions, goals and discharge summaries.
The selected candidate will obtains approval for admissions form the Director of HospiceCare regarding private insurance admission, non-reimbursed services, pediatric admissions and other special populations, and special needs of patients meeting the criteria for acceptance.
