The RN Charge Nurse is responsible for all safety, infection control, and quality of care issues that might arise while on duty, as well as, assisting with performance appraisals, mentoring staff during orientation, and dealing with disciplinary issues.
Identifies environmental factors which deter effective patient care, initiates action, and contributes ideas towards resolution; assesses patient and family needs and documents this information appropriately in the computer or on appropriate form.
This position melds both the duties of an HRCM RN and a Wellness Coach. Provides education and counseling to client employees identified as appropriate for any of the Health Risk and Condition Management (HRCM) programs offered by Premise Health. Works with employees one-on-one or in small groups to assist in meeting health and wellness goals related to weight, nutrition, exercise, stress, tobacco cessation, work/life balance and general health.
As a home care nurse, RN, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.
