As a home care nurse, RN, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.
Establishes the plan of care in collaboration with the physician, patient and UM Professional in accordance with the physician orders and patient needs. Advises and consults with family and other agency personnel, as appropriate. Completes documentation including visit assessment, OASIS, 485 and Verbal Orders accurately and promptly on the laptop device or in written documentation while in the patient's home and within the agency's and regulatory standards. Data syncs (transmits) information same day.
Provides direct patient care, evaluates outcomes and adjusts nursing care process as indicated to ensure optimal patient care. Initiates, coordinates and evaluates patient and family centered health teaching.
In this role, you will be responsible to provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer on-going care and case management for each patient, provide necessary follow-up as directed by the clinical manager.
Perform physical therapy assessment, reassessment and treatment and supervises physical therapist assistants in the treatment of pediatric, adolescent, adult, and geriatric patients referred by a physician to the clinic.
This position melds both the duties of an HRCM RN and a Wellness Coach. Provides education and counseling to client employees identified as appropriate for any of the Health Risk and Condition Management (HRCM) programs offered by Premise Health. Works with employees one-on-one or in small groups to assist in meeting health and wellness goals related to weight, nutrition, exercise, stress, tobacco cessation, work/life balance and general health.
Provides clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.
