Responsible for providing IV nursing care in the home or ambulatory setting, performing in-hospital discharge assessments and occasional IV hook-ups, teaching patients about their home infusion therapy, and providing ongoing monitoring of their treatment.
Our Bone Marrow Transplant Coordinators work together with our physicians in the evaluation, care and education of transplant patients and their families. Following patients throughout the course of treatment, the RN coordinator will ensure the seamless continuity of treatment plans and be a central resource for the healthcare team.
The unit-based Care Coordinator functions as part of an interdisciplinary team of physicians, nurses and other professionals to maintain continuity of patient care for all patient populations from admission to discharge and utilizes appropriate criteria for inpatient care and timely discharges.
Responsible for supervising nursing personnel to deliver nursing care and within scope of practice coordinates care delivery, which will ensure that patient's needs are met in accordance with professional standards of practice through physician orders.
In this role, you will be responsible to provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer on-going care and case management for each patient, provide necessary follow-up as directed by the clinical manager.
Provide skilled professional nursing care to patient/family/caregivers as prescribed by the physician and in compliance with regulations by the State Board of Registered Nursing and company policies when called upon after hours.
