In this role, you will assist with implementation of medication safety actions, interventions, and process changes (operational and clinical) to prevent ME's and ensure safe medication practices. Work collaboratively with Pharmacy Managers, Medical Director Patient Safety, Risk Managers, DUDSM, nursing, PharmIT team, E-record team and PharmNet team to proactively implement safe medication practices.
This position will ensure the provision of cost-effective pharmacotherapy contributing to optimal patient outcomes, provide experiential education for present and future health care practitioners, and support drug-related research. To provide observational oversight for the safe and effective compounding of medications.
In this role, you will be responsible for monitoring of drug therapy, communicating with all providers involved in the care of patients with respect to pharmacotherapy issues, participating in assuring adherence to Medical Center drug use policy, and providing pharmacotherapy education and information for physicians, nurses, and residents, students, and patients.
In this role, you will ensure the accuracy and appropriateness of all prescriptions filled by completing drug utilization review and final quality assurance, applicable to state and federal board of pharmacy regulations.
The Pharmacist will respond to questions from prescribers, nurses, pharmacy technicians , support staff and patients. The Pharmcist will provide friendly, appropriate counseling to a diverse patient population.
